Carers information

We would like to help identify as many carers as possible who are registered at the practice whether you or someone you know is a formal carer or helps to look after someone else. We would like to offer support to all our carers at the practice.

We aim to:

  • Identify and register carers to make it easier to offer practical things, (like health checks and flu vaccines) to maintain good health of carers.
  • Timely identification and referral of young carers can help reduce the impacts of inappropriate caring during childhood.
  • Having up-to-date carers register helps us to target health screening for areas of risk, such as depression, high blood pressure, etc.
  • Early identification of carer health problems can lead to faster treatment and improved health outcomes.
  • Improved support for the carer can lead to better care planning and more effective implementation of the subsequent care plan.
  • Improved physical health and emotional wellbeing of carers can lead to reduced demand on services.
  • We also have a social prescriber attached to the practice that can help support carers to access social events/local support groups to help to provide further advice and information to carers.
