Dear patients
As you are already aware, electronic record sharing across health and social care is really important to deliver quicker access, diagnosis and treatment for you and your family members, especially those patients who are coming into contact with other services (e.g. 111, secondary care, emergency departments, social care). If you are unsure of what electronic record sharing is please see below a list of relevant information that you will find useful and if you wish to sign up please print the form and hand in to reception. We advise all patients to sign up as it helps you. This is however not mandatory.
